What would you do with this 58 YO woman with fever and syncope? Well…she went through a chest pain protocol under supervision of cardiology, got diagnosed with a viral illness, and discharged. And she probably would have died had it not been for an astute emergency physician from Kuala Lumpur. What would you do? 
 

Brugada Syndrome

  • ECG Findings in V1-V3
    • RBBB or Incomplete RBBB
    • Coved or Saddle type ST-segment elevation
  • High mortality. May present with syncope or sudden death.
  • Responsible for up to 20% of sudden cardiac death in patients without any structural heart abnormalities.
  • Need EP study and likely ICD placement
