The worst audiobook I've ever listened to included a combination of these things. It was a librivox version of The Phantom of the Opera (read in November 2020).
There were so many different narrators and not all of them were great. I had to listen at 2x speed to make it more bearable. Some were monotonous, some were too far away from the microphone when they're reading and it would have been so much better if the whole book had just one narrator. Chapter 22 was the absolute worst and it is rather ironic that that chapter is called "In the Torture Chamber" because it had awful audio quality and was torture on my ears! I rated this book on the story only (4 stars). It is unfair for me to deduct stars for a poor bunch of narrators. I'm sure there are others out there who can do this classic story justice.
I voted for "does annoying voices for certain characters" because I have too much beef with the Discworld audiobooks. Though the real answer is that the Farseer trilogy is the worst for the way the American narrator puts on a bad British accent the whole way through. Unlistenable.

Rewatching episode 1 of Merlin and my god it sets so much stuff up so fast. I am only 10 mins in and we've already established that Merlin had to travel to a new town bc his mother felt he was unsafe, Uther's stance on magic, Morgana's unhappiness, Merlin saving Gaius's life and explaining that he was Born This Way, the monster of the week plot of the episode with Gwen from Torchwood, and the dragon. He hasn't even met Arthur yet!
